Residency education funding

How about shifting the reliance on medicare funding for GME (residency/fellowship) to private inititiaves such as hospital funded programs- after all its the hospitals who rely on residents talents and hard work to provide care to patients- is it too much to ask hospitals to foot some bills especially when the GME salary is already too low.

We have a significant shortage of not just primary care physicians in US but also in the area of specialists- hospitals/health care systems are stuck with long time for patients to be seen by a MD or else seen by a generalist who is not an MD (NP or PAs).

Its about time that we we shift the onus of GME funding and let the private sector be given greater opportunities at funding residency programs to have a sustainable pipeline of high quality physicians (not providers).
